1. Complete our Agency Questionnaire (PDF)
2. Return form to us along with the following information:
- A copy of your E & O Policy
- A copy of your California Insurance License
- Your financial statement
NOTE: If you prefer, we will accept a signed Personal Guarantee in lieu of Financial Statements.
After receipt and review of the above, we will forward to you our Agency Agreement and we can start accepting your submissions. We look forward to working with you!
